OH Medical Exams Blanket Purchase Agreement (BPA)

<p>The Virginia National Guard is looking for a vendor to provide medical exams to employees who may have been exposed to hazard materials.</p>

<p><strong>1. COMMUNICATION WITH THE GOVERNMENT:</strong></p>

<p>A. Questions: Submit written questions via email ONLY to the contract specialist Katina Moore at katina.l.moore7.<br />
civ@army.mil. No phone calls will be accepted. Questions will be received until <strong>October 9, 2026,<br />
no later than 11:00 a.m. Eastern Standard Time (EST).</strong></p>

<p><strong>2. ADDENDUM to 52.212-1, Instructions to Offerors -Commercial Items (SEP 2023)</strong><br />
&nbsp;</p>

<p>A. Addendum to FAR 52.212-2(b), Submission of offers. The offeror shall submit an electronic<br />
copy of their dated quote via e-mail to: katina.l.moore7.civ@army.mil with the solicitation number<br />
W912LQ-27-A001 in the subject block by <strong>October 19, 2026, no later than 11:00 a.m. EST.</strong><br />
&nbsp;</p>

<p>Facsimile quotes will not be accepted. Offerors who do not provide all the required information with<br />
their quotes may be deemed NONRESPONSIVE for further consideration for the solicitation and<br />
award. In addition to the items required by 52.212-1(a), the offeror shall complete and provide the<br />
following with their submission:</p>

1. Signed and completed Information Sheet. (Attachment 1)<br />
2. Completed Reps and Certs for 52.219-1<br />
3. Proof of medical liability insurance<br />
4. Pricing Schedule attached.<br />
