Scleral Contact Lenses for WAMC

<p>This is a combined synopsis/solicitation for commercial items prepared in accordance<br />
with the format in RFO FAR Subpart 12.2, as supplemented with additional<br />
information included in this notice. This announcement constitutes the only<br />
solicitation; quotes are being requested and a written solicitation will not be issued.</p>

<p>The Defense Health Agency Contracting Activity Healthcare Contracting Division -<br />
West, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, and award<br />
a Blanket Purchase Agreement (BPA) with Firm Fixed-Prices for Scleral Contact Lenses for Womack&nbsp;Army Medical Center (WAMC), Ft. Bragg, NC.</p>

<p>The solicitation number for this request for quotation (RFQ) is HT940626QE090. The<br />
solicitation document and incorporated provisions and clauses are those in effect<br />
through Federal Acquisition Circular 2026-01. The Standard Industrial Classification<br />
Code is 6540, size standard is 1000 employees and the NAICS code of 339115.</p>

<p>All responsible offerors shall provide an offer that meets the Statement of Work.<br />
See pages 4 and 5 of solicitation HT940626QE090.</p>

<p>The provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum<br />
applies to this acquisition. See pages 24 and 25 of solicitation HT940626QE090.</p>

<p>The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its<br />

<p>The Defense Priorities and Allocations System (DPAS) and assigned rating is not<br />
applicable.</p>

<p>The Government intends to award a Firm Fixed-Price BPA with an ordering period<br />
of 1 OCT 2026 - 30 SEP 2031 to the successful quoter.</p>

<p>Contract Requirements - The successful offeror shall be required to meet the<br />
requirements of the combined synopsis/solicitation attachment.</p>
