AMENDMENT 0002 - 19KU2026Q0012: Chancery Annex Restroom, Lactation RM &Locker RMs Renovation

<p><strong><u>Subject: Request for Quotation number RFQ 19KU2026Q0012 - RESTROOMS &amp; LACTATION ROOM UPGRADE PROJECT</u></strong></p>

<p>Enclosed is a Request for Quotations (RFQ) for the Chancery Annex Restroom, Lactation RM &amp; Locker RMs Renovation. If you would like to submit a quotation, follow all solicitation instructions, and complete the required portions of the attached document.</p>

<p>The U.S. Government intends to award a contract/purchase order to the responsible company submitting a technically acceptable offer at the lowest price. We intend to award a contract based on initial quotations, without holding discussions, although we may hold discussions with companies in the competitive range if there is a need to do so.</p>

<p><strong>Note:</strong> Prospective offerors are strongly encouraged, though not required, to register in the System for Award Management (SAM) at https://www.sam.gov.</p>

<ul>
<li>Offerors <strong>with an active SAM registration</strong> need only provide their Unique Entity Identifier (UEI) number and are not required to complete the information 52.204-90 since relevant information is found in their SAM registration.</li>
<li>Offerors <strong>without an active SAM registration</strong> must complete FAR 52.204-90, Offeror Identification, in Section 5, Representations and Certifications. This information must be updated annually if option years are included in the solicitation or award.</li>
</ul>

<p>Offerors are strongly encouraged to maintain an active SAM account</p>

<p><strong><u>For a quotation to be considered, you must also complete and submit the following:</u></strong></p>

<ol>
<li>SF-1442 (14 through 20c)</li>
<li>Section A, Price</li>
<li>Section L, Representations, certifications, and other statements of offerors or quoters</li>
<li>Information and documentation required by Section J, Attachment A, and all other applicable provisions of this solicitation.</li>
<li>Proof of SAM Registration / Unique Entity ID, or if not SAM registered, complete FAR 52.204-90, Offeror Identification,</li>
</ol>

<p>Failure to follow these instructions will render the offer non-compliant and ineligible for further consideration</p>

<p><strong>The Purpsose of this amendment 0002 is to:</strong></p>

<p><strong>1. Post Questions and Answers; and</strong></p>

<p><strong>2.&nbsp;</strong>Attachment A &ndash; Statement of Work is hereby deleted in its entirety and replaced with the revised Attachment A &ndash; Statement of Work, attached to this amendment..&nbsp;The revised Attachment A incorporates applicable technical clarifications and revisions resulting from the Government&#39;s responses to offeror questions, including, but not limited to, clarifications concerning room dimensions, drywall and moisture-resistant material requirements, tile and waterproofing requirements, counters and sinks, plumbing and drainage, shower and hand-sprayer requirements, ABA/ADA requirements, material and product submittals, project documentation, safety plans, warranty requirements, and other technical requirements addressed in the Questions and Answers.</p>

<p>3.&nbsp;Offerors shall prepare their quotations based on the solicitation as amended, including the revised Attachment A.</p>

<p><strong><u>All other terms and conditions of the solicitation remain unchanged.</u></strong></p>
