<p><u><strong>GENERAL</strong></u><br />
This is a combined synopsis/solicitation for commercial products or commercial services<br />
prepared in accordance with the Revolutionary Federal Acquisition Regulation Overhaul (RFO)<br />
part 12. This announcement constitutes the only solicitation. Offers are being requested and a<br />
separate written solicitation will not be issued.</p>

<p>Solicitation number <strong>FA664326Q0004</strong> is issued as a Request for Quote (RFQ) for non-personal<br />
Aeromedical Case Management support services in accordance with the attached Performance<br />
Work Statement (PWS).</p>

<p>This acquisition is set-aside for Small Business concerns (Total 100%). This solicitation<br />
incorporates provisions and clauses by reference. The full text of provisions and clauses may be<br />
accessed electronically at www.acquisition.gov.</p>

<u><strong>RESPONSE TIMES</strong></u><br />
<strong>Quotations</strong> must be submitted electronically via email to the Contracting Officer at<br />
bianncia.glay@us.af.mil no later than the date and time specified in this solicitation <strong><u>(07 August<br />
2026, 4:00 PM EST).</u></strong> The email subject line shall read: &ldquo;Quote Submission: Aeromedical Case<br />
Management Support &ndash; [Insert Offeror Name]&rdquo;.</p>

<p><strong>Questions</strong> must be submitted electronically via email to the aforementioned Contracting Officer.<br />
Note, questions must be submitted no later than <strong>Friday, 31 July 2026</strong> by close of business.</p>

<p><u><strong>PERIOD OF PERFORMANCE</strong></u><br />
The anticipated Period of Performance consists of a 12-month Base Period and four (4)<br />
subsequent Option Periods.</p>

<p><strong>QUOTATION SUBMISSION: </strong>Interested Offerors shall submit quotations in accordance with<br />
the attached SF1449. Detailed quotation preparation instructions, submission requirements, and<br />
evaluation procedures are contained in the Addendum to RFO 52.212-1, Instructions to<br />
Offerors&mdash;Commercial Products and Commercial Services, and the Addendum to RFO 52.212-<br />
2, Evaluation Factors for Award. Offerors are responsible for reviewing the complete solicitation<br />
package and complying with all instructions and requirements contained therein.</p>

<p><strong>BEST AND FINAL SUBMISSION: </strong>Offerors must submit their best and final quotation. The<br />
Government intends to evaluate quotations and make award without negotiations. Accordingly,<br />
offerors are encouraged to submit their most favorable technical and pricing terms in their initial<br />
quotation.</p>

<p><u><strong>DESCRIPTION</strong></u><br />
The Department of the Air Force, Air Force Reserve Command (AFRC), intends to award a<br />
competitive commercial services contract for non-personal, administrative, and aeromedical case<br />
management support for the Headquarters Air Force Reserve Command (HQ AFRC) Office of<br />
the Command Surgeon. This acquisition is being conducted as a commercial services acquisition<br />
using the policies and procedures of RFO Part 12.</p>

<p>The Contractor must provide a comprehensive, turnkey, non-personal services solution,<br />
including all management, personnel, operational oversight, and other resources necessary to<br />
execute high-volume medical case processing, accurate data entry into DoD/Air Force medical<br />
systems of record (e.g., AIMWTS, ECT, PEPP), program administration, and unit-level customer<br />
support for approximately 70,000 Air Force Reserve personnel. The contractor must manage all<br />
workflows to consistently meet the performance thresholds established in the Services Summary<br />
of the attached PWS, including all workload fluctuations up to the 15% surge threshold.</p>

<u><strong>CONTRACT LINE-ITEM STRUCTURE</strong></u><br />
0001 - Aeromedical Case Management Support Services (Base Period) &ndash; 12 Months<br />
1001 - Aeromedical Case Management Support Services (Option Year 1) &ndash; 12 Month<br />
2001 - Aeromedical Case Management Support Services (Option Year 2) &ndash; 12 Month<br />
3001 - Aeromedical Case Management Support Services (Option Year 3) &ndash; 12 Month<br />
4001 - Aeromedical Case Management Support Services (Option Year 4) &ndash; 12 Month<br />
<em>* Note for all CLINs: The Contractor must provide a Firm-Fixed-Price (FFP) for one (1) month of<br />
continuous support. The monthly price must encompass all labor, management, and operational oversight<br />
necessary to successfully perform all tasks outlined in the attached PWS. Invoicing is authorized monthly<br />
in arrears.</em></p>

<p><u><strong>APPLICABLE SOLICITATION PROVISIONS AND CLAUSES</strong></u><br />
This solicitation incorporates all applicable solicitation provisions and contract clauses identified<br />
in the attached Standard Form 1449 and any accompanying solicitation attachments. Offerors are<br />
responsible for reviewing the complete solicitation package, including all incorporated<br />
provisions and clauses.</p>
